The challenges and facilitators of implementing the Antimicrobial Stewardship Program in hospitals: a literature review

Objective: To describe the state of the art of Antimicrobial Stewardship Programs (ASP) worldwide, to identify the main facilitators and challenges to program implementation, and to understand the role of pharmacists in this process. Methods: This is a literature review. Searches were conducted in the PubMed, LILACS, and Scopus databases. The MeSH/DeCS descriptors “antimicrobial stewardship” and “hospital” were combined with the keywords “barriers” and “facilitators.” Study selection and data extraction were performed independently by researchers. Results: A total of 188 studies were identified, of which 41 were included. Lack of time and high workload were the main challenges reported for ASP implementation, while the use of technology was highlighted as the main facilitator. Corroborating these findings, pharmacists emerged as professionals frequently associated with program success due to their expertise in medications. Conclusion: The main facilitators and challenges identified in the implementation of hospital-based ASPs involve human resource management and investment in technology. Pharmacists play a relevant role in program management.
