Objectives: This study investigates the profile of users of the Melhor em Casa Program (PMC) in the municipality of Bento Gonçalves, Rio Grande do Sul, between 2018 and 2022. The PMC is a home care initiative aimed at providing humanized and cost-effective healthcare for patients with mobility issues, chronic illnesses, and terminal conditions. This paper aims to describe the demographic and clinical characteristics of the users, analyze clinical outcomes, and identify factors associated with discharges and deaths. Methodology: This is a descriptive quantitative study, with retrospective data collection from municipal management systems and electronic medical records, covering 510 users. The data were analyzed using frequencies, percentages, and logistic regression to predict clinical outcomes. Results and conclusion: The results indicate a predominance of elderly, white patients, mainly females, highlighting the importance of PMC in managing chronic and palliative care and the need for inclusive public policies that ensure equity in access to healthcare services.
