A hipertrofia adenoideana e sua relação com distúrbios psiquiátricos e neurocognitivos em uma população pediátrica

Introduction: Adenoid hypertrophy is the most common cause of upper airway obstruction in children and can be easily treated by adenoidectomy. In this population, mouth breathing can result in different types of sleep disorders, emotional instability, neurocognitive dysfunction, hyperactivity, conduct disorders, among other psychiatric issues. Objective: To evaluate the prevalence of psychiatric and neurocognitive disorders in children and adolescents undergoing adenoidectomy and to analyze the clinical-epidemiological characteristics of this population. Material and methods: Cross-sectional study, involving patients undergoing adenoidectomy at the University Hospital of the Federal University of Juiz de Fora (HU-UFJF), between March 2021 and December 2023. Results: 102 patients were recruited with a mean age of 8.78 years, with a standard deviation of ± 3.16 years. All patients had surgical indications, with 22.5% of them previously diagnosed with a psychiatric or neurodevelopmental disorder. The most prevalent disorder was ADHD, representing 30.1% of the total sample with a previous diagnosis. The prevalence of disorders was similar in both sexes, being 11.8% in males and 10.8% in females. Discussion: In the present study, ADHD was the most prevalent disorder, representing 30.1% of patients diagnosed with some disorder prior to surgery. Adenoidectomy, when performed in children with severe upper airway obstruction, benefits the respiratory pattern and can improve attention, behavioral disorders, cognitive functions, in addition to reducing the need for the use of psychiatric medications compared to the pre- operative. Conclusion: Psychiatric and neurocognitive disorders have a significant prevalence in the pediatric population with adenoid hypertrophy. Such disorders must always be investigated in this kind of patients and, in case of diagnostic confirmation, multidisciplinary treatment must be instituted.
