Association between oral lichen planus and hepatitis C: case report

Introduction: Hepatitis C (HCV) is a public health problem with silent symptoms. Its infection can develop with extrahepatic manifestations of an autoimmune character, due to the formation of immune complexes. One of these manifestations is oral lichen planus (OLP). Objective: Report a case of concomitance between OLP and HCV, demonstrating the importance of the dentist in detecting systemic diseases through oral repercussions. Case report: A 65-year-old female patient sought dental treatment with the main complaint of pain in the cheek mucosa lasting 3 months. On extraoral examination, jaundice was found in the eyes, while on intraoral examination, white lesions with a reticulated and striated aspect were detected in the cheek mucosa, bilaterally of approximately 1 cm on each side, diagnosed as OLP. Anemia and HCV were diagnosed through laboratory blood tests. Clobetasol propionate 0.05% was prescribed to treat oral lesions for mouthwash three times a day for 15 days, with complete remission of the lesions. The patient was referred for concomitant treatment with a hepatologist. Conclusion: It is important that, during the consultation, the dentist requests additional tests, such as the anti-HCV test, and investigates risk factors and signs suggestive of chronic liver diseases, especially in patients with OLP.
