Identification of mandibular foramen height in panoramic radiography as an auxiliary method in anesthesiology

Abstract

Introduction: The mandibular foramen (MF) is an important anatomical reference for the effective blocking of the inferior alveolar nerve (IAN). The lack of knowledge of the presence and anatomical variations of this foramen may have unfavorable implications, such as complications in the performance of the procedures, as well as failure in the anesthetic technique. Aim: To establish an average between the distance from the second occlusal molar to the mandibular foramen, through the evaluation of orthopantomograms, with the purpose of reducing the failure of IAN anesthesia and the painful symptoms in mandibular dental procedures. Material and Methods: Retrospective observational study, in which 60 orthopantomograms of 16- to 40-year-old male and female patients with lower second molars were analyzed. The average distance between these teeth and the left and right mandibular foramina was determined by measurement using the Foxit Reader program. Results: The observed average was 11.2mm on the left and 10.89 on the right, and this difference was statistically significant at p <0.05. The older the patient, the shorter the distances between the lower second molar and the mandibular foramen on both sides. Regarding gender, males tend to have greater distances than females. Conclusion: The identification of the MF, through the analyzed orthopantomograms, contributes to the accomplishment of the IAN blocking in dental procedures, through the identification of the average height of this foramen.
