CARE SCENARIO ESTABLISHED FOR DEPENDENT ELDERLY PEOPLE, WHO WERE DISCHARGED FROM THE HOSPITAL

Abstract

The objective of this study was to describe the care scenario established for dependent elderly people, who were discharged from the hospital, and are residents in areas confined to a Basic Health Unit with Family Health Strategy and Home Care Service in the city of Juiz de Fora, Minas Gerais, from April 1, 2017 to March 31, 2018; and furthermore, to identify the number of elderly dependent on home attention and care after discharged from the hospital in area with nursing coverage and multiprofessional teams of home care. Descriptive, descriptive and exploratory quantitative study using secondary data extracted from the hospital information system, called: e-"SUS" were analysed. Charts and worksheets of home care service / "The Program called "Better at home", related to the elderly registered by teams of the city. Participants of the study were users of the Unified Health System, 60 years old people or over 60 years old people, living on streets near a Basic Health Unit with a Family Health Strategy, which had already been discharged from the hospital during the study period. They were bedridden with functional incapacity and demanding nursing care in the three "home care modalities". The collected data were organized and analyzed descriptively, being presented in the form of tables, graphs and boards; being discussed based on the ideas of SUS and published researches on the subject. The results showed that, compared to the administrative area, the northern region of the city had the highest number of elderly people who were hospitalized for some time and were discharged during the study period. From the total of 6,825 hospital discharges from the elderly aged 60 or over, 5,486 of whom were in primary care areas and 3,605 (5.82%) were female. Among the 89 elderly people enrolled in the home care service, most of them were between 70 and 89 years old; the greater number of elderly people who demanded the Home Care Service were more than 70 years old; the majority was referenced by the Basic Health Unit (56.7%) and 43.6% remained active. Of the 289 elderly, 89 were discharged from hospital and were admitted / accompanied in the Home Care Service.
